We have monitored the service for stability and this incident shall now be closed.

Monitoring

Broadcom Engineering has implemented a fix for this issue and have confirmed that new users are now being created as expected. We will continue to monitor this issue across the next day.

Investigating

Broadcom Engineering is investigating an issue related to failures while creating new users. If a user experiences this problem while creating a new user in the AppNeta system, they will receive an "Internal Server Error" error message at the top of the form.

We will post updates here as we investigate.
